Skilled Nursing

Our Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) provide individualized care in the comfort of your own home. We work in collaboration with your physician to monitor and supervise  the implementation of your care plan based on your own unique need. Aside from providing you with the needed skilled nursing services, our services extend to but do not limit to the following:

Occupational Therapy

 We aim for our patients to reclaim occupational tasks after the advent of disease or injury. Patients who are not well-prepared to be reintroduced to daily living tasks are easy candidates for relapse and further health risks. \

All Access Home Health Inc provides occupational therapy services for the rehabilitation of basic living skills of patients recuperating at home. Occupational therapy focuses on engaging the individuals involved in a series of daily living activities; gradually reorienting them physically and emotionally to the targeted skills. Occupational therapists are responsible for conducting assessments and evaluations on a baseline of retained, attained, and to be obtained skill set criteria along with a specified timeframe.
Occupational therapists focus on:

Speech Therapy

Our licensed and experienced therapists specialize in creating personalized therapy plans for your loved one based on comprehensive evaluations that take into consideration unique medical, cultural, and linguistic backgrounds. Our SLP’s are specifically trained for the following:

  • Auditory Processing Disorders Therapy

  • Dysphagia Treatment (Difficulty in Swallowing)

  • Assessment and Recommendation for Learning Disabilities

  • Instruction on Eating and Swallowing Strategies

  • Treating Fluency Difficulties

​Speech Therapy and/or Language Pathology will be provided to clients in their homes or their preferred living environment. We will adjust the inclusions of services based on your care plan – which will be developed by our care coordinator and reviewed by your physician.

Home Health Aide

Home Health Aides will assist your loved one with activities of daily living and personal care:

​

  • Assisting the Client with Feeding / Eating

  • Transportation Arrangements to Medical Appointments

  • Preparing Meals (According to Client’s Dietary Specifications)

  • General Monitoring of Health Conditions

  • Help the Client with Daily Personal Tasks (Bathing and Dressing)

  • AM and PM Care

  • Bathing Assistance
     

Home Health Aide Services will be provided to clients in their homes or their preferred living environment. We will adjust the inclusions of services based on your care plan – which will be developed by our care coordinator and reviewed by your physician.

 

Medical Social Worker

 

Medical Social Workers will assist the family members and the client in securing continuity of care after hospital discharge:

  • ​Coordinate Follow-up Treatments or Doctor Visits

  • Facility-Based Care Placement

  • Private Counseling Services for the Patient

  • Facilitating Advance Directives (Living Will or Power of Attorney)

  • Supportive Case Work

  • Community Resource Coordination

  • Planning Alternative Living or Supported Living

  • Assistance with Financial Aid Applications
